Select desired statement template to be used. Statement templates are defined using the Statement Templates program.
The 
button can be used to open the Statement
Template Lookup window that allows you to view a list of statement templates and select any statement
template by double-clicking it.
The OK button will select the specified statement template.
The Cancel button will close the Select Statement Template window without selecting a template.
